In order to participate in Giant Steps Therapeutic Riding Program, potential riders or family members must complete the application forms below, including a physician assessment & release.

Please mail completed forms to P.O. Box 4855, Petaluma, CA 94955.  You will be contacted for an assessment once all forms have been received.
